The City of Bangor Public Health is teaming with Penobscot Community Health Center to offer a COVID vaccination clinic at JFDS and WSCS for students aged 12 or older on May 21.

To arrange for your JFDS or WSCS student to participate in the clinic:

Review the attached Pfizer COVID-19 Vaccine Fact Sheet.

Submit the appropriate Consent Form to the Main Office as soon as possible.

Complete the pre-registration form, which will assist clinic planning.
