Dear Parents/Guardians,
The Bangor School Department in collaboration with Bangor Public Health will be offering vaccination clinics to administer the COVID-19 Booster Vaccine for children 12 years of age and older. Children will be offered the Pfizer-Biotech COVID-19 Booster Vaccine to prevent coronavirus disease 2019(COVID-19) caused by SARS-CoV2. The booster is available to those individuals 12 and older who have had their complete vaccine series (2 shots) AND it has been over 5 months since the last vaccine. The clinic will also provide opportunity for those wishing to begin the vaccination series of two shots.
The clinic will be offered on Tuesday, February 15, 2022 from 3-5pm at both the James F. Doughty School and William S. Cohen School. In order to prepare for the clinic, we ask that you call the school in which you will attend the clinic and sign up for a spot no later than Friday, February 11, 2022. We will stagger appointments every 10 minutes and will offer six slots every ten minutes at both sites. Please call the schools at the number below

Please be sure to bring your signed consent form with you to the clinic. PLEASE NOTE THAT THE CONSENT FOR THE INITIAL VACCINATION IS DIFFERENT FROM THE BOOSTER.
